:Introduction

Kidneys are very important organs. They clean the blood and regulate the fluids in the body. Kidneys are vulnerable to many diseases. Some kidney diseases are life threatening, and others can lead to complete kidney failure and require dialysis.

This reference summary explains how the kidneys work, as well as a serious disease of the kidneys called endـstage renal disease, or ESRD.

:Anatomy

Kidneys are beanـshaped organs located in the middle to lower back, on both sides of the spine. The urine that is formed in the kidneys flows through tubes, called ureters, to be stored in the bladder.

When the bladder is full, a person feels the urge to urinate. Urine is emptied through the urethra. The main purpose of the kidneys is to make urine by filtering harmful chemicals, called toxins, out of the blood. The kidneys also help to keep necessary chemical substances in the blood, such as sodium, proteins, and sugars.

The kidneys control the amount of fluid we keep in our bodies. For example, when we drink more fluids then we need, the kidneys secrete extra urine. On the other hand, when we do not drink as much fluid as we need, the kidneys make less urine. The kidneys help to regulate hormones that strengthen our bones and produce red blood cells. Since the kidneys constantly filter the blood, they are very sensitive to anything we eat or drink, or to any medications we take.

The kidneys also help to keep our blood pressure regular; this is why many kidney diseases result in high blood pressure.

Damage to only ١ kidney is not usually a major problem, since the other kidney can take over for it. However, if both kidneys get damaged, a person will die within a few days if they do not get dialysis.

:ESRD

ESRD, short for endـstage renal disease, develops when both kidneys are not able to function. Sometimes it happens gradually and sometimes all at once. The following are diseases that can lead to ESRD:

  • diabetes
  • hypertension
  • lupus
  • repeated kidney infections
  • kidney stones
  • kidney cysts
  • infection in the blood called sepsis

The constant use of painkillers, alcohol, or other medications can also lead to ESRD.

A kidney specialist, called a nephrologist, can sometimes help to slow the development of ESRD. This is usually done using medications and changes in the patient’s diet.

It is very important to have kidney function checked regularly if you:

  • have a disease that could lead to ESRD or
  • take any medication regularly

To check on how the kidneys are functioning, blood tests and urine tests are needed.

:Signs & Symptoms

When the kidneys are not working right, the body retains water, which leads to swelling in the face, ankles, legs, and throughout the body. The extra fluid can cause shortness of breath. Since toxins build up in the body when the kidneys are not filtering them out, patients with ESRD feel very tired and weak. They have no stamina or energy. This is partly due to the body not producing enough red blood cells, a condition called anemia.

Kidney failure patients sometimes feel pain below the rib cage. As kidney failure gets worse, blood pressure tends to rise and the body makes less urine.

With time, kidney failure causes the skin to become pale. If nothing is done to treat kidney failure, death can result from toxins building up in the body, as well as high levels of potassium in the blood.

:Treatment

The main treatment for ESRD is dialysis. Dialysis is a process that replaces the function of the kidneys. More than ٣٠٠,٠٠٠ patients in the USA depend on dialysis in order to stay alive. In some cases, dialysis is temporary, and can be stopped as soon as the kidneys are able to function again. However, dialysis is usually a lifetime treatment.

There are ٢ types of dialysis: hemodialysis and peritoneal dialysis. Each works differently to filter toxins out of the blood, as the kidneys normally do. Hemodialysis reroutes the blood to a dialysis machine. The machine cleans toxins from the blood and regulates the levels of essential chemicals, such as potassium.

Hemodialysis is done ٣ـ۴ times per week and each session takes around ۴ـ۵ hours. Special IVs are needed for good blood flow to and from the dialysis machine. Peritoneal dialysis uses the peritoneum, which is the lining of the abdominal cavity, to clean the blood. A tube, also known as a catheter, is surgically placed in the peritoneal cavity. A solution called “dialysate” is allowed to flow into the peritoneal cavity through it the catheter.

The dialysate solution interacts with the blood through the peritoneum, filtering out toxins and cleaning it over a period of several hours.

After peritoneal dialysis, the fluid in the peritoneum is drained to the outside of the body into a special bag. The same cycle is repeated many times a week; it is sometimes started before the patient goes to bed and the fluid emptied in the morning.

For dialysis to be successful, patients have to follow strict dietary restrictions on the amount of salts, proteins, and fluids they eat and drink.

A kidney transplant can cure some cases of ESRD. Unfortunately, there are not enough kidneys available for all ESRD patients. In addition, a kidney transplant is not the best option for every ESRD patient. There are currently ٣۵,٠٠٠ people on kidney transplant waiting lists!

Prevention:

People who are at risk of getting ESRD should be closely monitored by a family doctor or nephrologist for early signs of it. People who have any of the following conditions are at risk of ESRD:

  • known kidney diseases:
  • diabetes
  • hypertension
  • lupus

Some dietary restrictions may be needed to slow down and possibly stop the process that leads to ESRD. Some of these restrictions include eating less :

  • (salt (or sodium
  • potassium
  • proteins

Other general healthy lifestyles changes may also help, including:

  • regular exercise
  • not smoking
  • maintaining an ideal weight

Kidneys are extremely important organs that filter harmful substances out of the body. The kidneys are susceptible to many diseases, some may be life threatening. Others can lead to complete kidney or renal failure, requiring dialysis. Endـstage renal disease, or ESRD, is a very serious condition. Thanks to recent medical advances, patients with ESRD can live normal lives within the restrictions of their dialysis schedule.

Kidney transplant, if available and appropriate, can cure kidney disease.

The role of the patient in making lifestyle changes and keeping up with the dialysis schedule is very important for successfully treating ESRD.

مقدمه:

کلیه ها اعضای بسیار مهمی در بدن هستند که مسئول تصفیهی خون و تنظیم مایعات در بدن هستند.کلیه ها در برابر بسیاری از بیماری ها آسیب پذیرند.بعضی از بیماری های کلیوی زندگی را تهدید میکنند و برخی دیگرآنرا به خطر می اندازند و باعث میشوند که نارسایی کامل کلیه اتفاق بیوفتد وشخص نیازمند انجام  دیالیز شود. 
این  مبحث قصد دارد چگونگی عملکرد کلیه ها و همچنین بیماری حاد کلیوی بنام
(ESRD (end-stage Renal Disease

  آناتومی :

  کلیه ها دو عضو لوبیایی شکل هستند که در دو طرف ستون فقرات پشت حفره شکمی قرار گرفته اند. ادراری که در کلیه ها تشکیل میشود در طی لوله هایی که برای حمل آنها تعبیه شده است به سمت مثانه جریان پیدا میکند تا درمثانه ذخیره شود.هنگامیکه مثانه پرشد شخص احساس دفع میکند و ادرار از طریق مجرای ادرار دفع میگردد. مهمترین وظیفه کلیه ها تصفیه ی شیمیایی خون از طریق تولید ادرار است. کلیه ها همچنین به تعادل میزان مواد شیمیایی موجود در خون ازقبیل سدیم,پروتئین ,و قند کمک میکنند. حجم مایعات بدن را تنظیم میکنندبرای مثال وقتی مایعات زیاد مینوشیم احساس نیاز به دفع بیشتری داریم . از سوی دیگروقتی که میزان مایعات کمتری به بدن وارد شود کلیه ها هم ادرار کمتری تولید میکنند. کلیه ها هورمونها یی را کنترل میکنند که موجب تقویت استخوانها و تولید گلبولهای قرمزمیشوند.

از آنجا که کلیه ها خون را بطور دقیق تصفیه میکنند نسبت به اینکه چه خورکی ها وداروهایی که مصرف میکنیم بسیار حساس هستند. کلیه ها در تنظیم فشار خون نقش مهمی دارند به همین دلیل است که بسیاری از بیماری های کلیوی درفشار خون بالا بوجود می آید.
چنانچه تنها یکی از کلیه ها آسیب ببیند مشکل حادی بوجود نخواهد آمد تا آنجا که کلیه ی دیگر بتواند وظایف را انجام دهد با این حال اگر هردو کلیه در شخصی آسیب ببیند, در صورت انجام ندادن دیالیز شخص از بین میرود.

ESRD:

مرحله ی پیشرفته یا به اصطلاح مرحله ی نهایی پیشرفت بیماری کلیه .این مرحله زمانی پیشرفت میکند که هردو کلیه ها عملکرد درستی نداشته باشند. گاهی اوقات این اتفاق بتدریج رخ میدهد و گاهی خیلی سریع اتفاق می افتد .

موارد زیر بیماری هایی هستند که ESRD را تشدید میکنند:

  1. دیابت
  2. فشار خون
  3. عفونت های مکرر کلیه
  4. سنگ کلیه
  5. کیست کلیه
  6. لوپوس (نوعی بیماری خود ایمنی)
  7. وجود عفونت در خون که سپسیس نامیده میشود

استفاده ی مداوم از مسکن ها,الکل یا بقیه ی داروها یی که کیتواند منجر به ESRD شود.
متخصص کلیه که نفرولوژیست نامیده میشود(متخصص بیماری های داخلی کلیه)میتواند گاهی اوقات به کند شدن پیشرفت ESRD کمک کند. معمولا با تجویز دارو یا تغییر رژیم این کار صورت میگیرد.

بررسی چگونگی عملکرد کلیه به طور مرتب  بسیار حائز اهمیت است اگر شما:

  • بیماری داشته باشید که احتمال ESRD داشته باشد
  • شخصی که مصرف داروهای متفاوت بطور منظم  دارد
  • بررسی عملکردکلیه ها توسط آزمایش خون و ادرار

علائم و نشانه ها:

زمانیکه کلی ها بدرستی کار نمیکنند بدن آب را نگه میدارد ,ورم در صورت ,زانوها و پاها دیده میشود. مایعات اضافی همچنین باعث تنگی نفس میشوند.زمانیکه کلیه ها نمیتوانند عملیات فیلتر کردن را به خوبی انجام دهند سموم در بدن تجمع پیدا میکنند.

بیماران دچار ESRD احساس ضعف و خستگی زیادی میکنند ,استقامت و انرژی ندارند دلیل این موضوع این است که بدن نمیتواند گلبول قرمز به میزان کافی تولید کند و شخص دچار بیماری کم خونی یا آنمیا(ANEMIA) میشود.بیمار دپار نارسایی کلیه گاهی درد درناحیه ی زیرین قفسه سینه حس میکند. هنگامی که نارسایی کلیه پیشرفت کند, فشار خون بالا میرود و ادرار کمتری تولید میشود.

نارسایی کلیه باعث میشود که پوست حالت رنگ پریدگی داشته باشد.اگر هیچ درمانی برای نارسایی کلیه صورت نگیرد میزان سموم تولید شده دربدن همچنین پتاسیم موجود در خون موجب مرگ شود.

درمان:

درمان اصلی ESRD دیالیز است که سیستمی است که جایگزین عملکرد کلیه میباشد . بیشتر از 300,000 بیمار در آمریکا از طریق دیالیز زندگی میکنند در بعضی موارد دیالیز موقتی است و میتواند متوقف شود و کلیه ها به عملکرد دوباره برگردند. دیالیز یک درمان حیاتی محسوب میشود.

2نوع دیالیز وجود دارد:

  • همودیالیز
  • دیالیز صفاقی

که هرکدام از آنها در تصفیه ی خون از سموم بطور متفاوتی بر اساس عملکرد طبیعی کلیه عمل میکنند. همودیایز خون را به دستگاه دیالیز میفرستد ,ماشین دیالیز خون را از کلیه ی مواد شیمیایی غیرضروری و پتاسیم پاک میکند و سموم را دفع میکند . همودیالیز میتواند 3-4 مرتبه در هفته هربار به مدت 4الی 5 ساعت انجام پذیرد. برای این کار بخصوص وریدهای سالم نیاز است که ورود و خروج خون به دستگاه را انجام دهد.

در دیالیز صفاقی از پریتونیوم (صفاق)استفاده میشود . در حفره ی شکم به وسیله ی جراحی یک لوله که کاتتر نامیده میشود کار گذاشته میشود ومحلولی که مایع دیالیز نامیده میشود اجازهدارد به وسیله ی این کاتتر به حفره ی شکمی وارد شود.

مایع دیالیز از طریق حفره شکمی با خون تبادل مواد انجام می دهد. سموم را فیلتر کرده ودر خلال چند ساعت این تصفیه انجام می شود. بعد از دیالیز صفاقی مایع دیالیزکه در داخل صفاق باعث تبادل مواد شیمیایی شده به سمت کیسه مخصوص هدایت میشود.این چرخه چند نوبت در طول هفته انجام میشود گاهی قبل از به رخت خواب رفتن بیمار شروع شده و پس از بیدارشدن وی مایع از شکم کاملا خارج میشود. برای داشتن یک دیالیز, موفق بیمار باید  رژیمی را رعایت کند که در آن مقادیر نمک , پروتئین و مایعاتی که خورده یا آشامیده میشود کنترل شده باشد.

پیوند کلیه برای بیماران ESRD راه نجات خوبی است اما متاسفانه تعداد کلیه های اهدایی به اندازه ی کافی نیست. بعلاوه پیوند کلیه همیشه بهترین راهبرای همه ی بیماران نیست . در حدود 35000 نفر در لیست انتظار پیوند کلیه هستند.

پیشگیری:

افرادی که خطر ابتلا به نارسایی کلیه ر دارند باید توسط نفرولوژیست متخصص کلیه ویزیت شده  و تحت نظر باشند .

افرادی که در خطرابتلا هستند:

  1.  بیماری شناخته شده کلیوی
  2. دیابت
  3. فشارخون
  4. لوپوس

یک سری رژیم های بازدارنده برای کند کردن روند بیماری یا متوقف کردن آن وجود دارند. تعدادی از این باز دارنده ها:

  1. نمک(سدیم)
  2. پتاسیم
  3. پروتئین

تغییراتی که  در سبک زندگی میتواند کمک کننده به کنترل این بیماری باشد:

  1. ورزش منظم
  2. عدم استعمال دخانیات
  3. نگه داشتن تناسب اندام و وزن ایده آل

خلاصه:

کلیه ها از مهمترین اعضایی هستند که مواد مضر را فیلتر میکنند  و به بیرون از بدن دفع میکنند.
کلیه ها مستعد بیماری های بسیاری هستند که میتوانند زندگی انسان را تهدید کنند و شخص را بسوی نارسایی کلیه و دیالیز سوق دهند.
ESRDیا آخرین حد پیشرفت بیماری کلیوی ,وضعیت بسیار جدی است اما  بیماری که با درجه حاد از این بیماری است میتواند زندگی نرمالی را بار برنامه ریزی دقیق دیالیز داشته باشد.
پیوند کلیه هم میتواند در بیماران کلیوی نجات دهنده باشد نقش بیمار در تغییر سبک زندگی اش و پیگیری برنامه دیالیزدر درمان موفقیت آمیز ESRD  بسیار مهم است.
